Please note: Insurance participation may vary by provider and specific insurance plan. Not all product lines are considered in-network. Please contact our office or your insurance carrier to verify eligibility and network participation prior to scheduling your appointment.

HMO Plans & Referrals

Certain HMO and Medicare Advantage plans require prior authorization and/or a referral from your primary care physician before scheduling an appointment.
Examples may include:

• Sutter Medical Group
• Providence Health Medical Network
• Imperial Health Plan
• Meritage / Amada Health

If your insurance plan requires a referral to see a specialist, please obtain it prior to your appointment to avoid delays or denied coverage.

Please Note:

We do not participate with:

• Kaiser Permanente
• Anthem Blue Cross Pathway / EPO plans
• Anthem Pathway PPO / EPO products
• Most Medi-Cal plans
• Workers’ Compensation plans

Appointment types

• Medical Dermatology – Office visits for medical skin concerns covered through your medical insurance.
• Telehealth – At this time, Dr. Julie Plante, Dr. Dale Westrom, and Dr. Susan Amaturo are offering in-person appointments only and do not currently provide telehealth visits.
• Cosmetic Consultations – Cosmetic consultations are not covered by medical insurance. Consultation fees are either $100 or $200. If you choose the $200 consultation, the fee will be applied toward the cost of your first treatment. The $100 consultation fee is non-refundable and not applied toward treatment costs. Transparent pricing for recommended procedures will be discussed during your visit. Procedure costs are separate from the consultation fee. In most cases, cosmetic services are scheduled after the in-person consultation.
• Surgical Consultations – Surgical consultations are medical office visits intended to evaluate the lesion under consideration. Procedures are scheduled after the in-person consultation and are not performed on the same day.

Registration

After scheduling your appointment, you will receive a registration link from eDerm. Please log in using the patient’s last name and date of birth. If you do not receive the email, please check your spam or junk folder.

Established patients should see a “Welcome Back!” message after validating their information. If you are an existing patient and do not see this message, please contact our office to avoid the creation of a duplicate chart.
